Center for Older Adult Services
The Center for Older Adult Services arranges, coordinates and monitors customized care plans for you or your aging relative. This innovative program helps older adults live meaningful and independent lives as long as possible. Our devoted staff will guide you and your family through difficult transitions, and help you find the best possible solutions to care for a loved one's changing needs.

We're here for you when...

  • You feel overwhelmed trying to care for your loved one's needs.
  • Isolation is making it hard to stay connected to friends and community.
  • You need help understanding which services are best suited for you or your aging relative.
  • Family relationships are strained because of the ongoing needs of your aging parent.

Your personalized plan of care will include an in-depth assessment of your relative's health status, home safety, cognitive functioning, family and community support, and more. With that information, our care manager will develop, implement, and monitor a customized care plan, matching your relative's needs with a wide range of services that support independence.

Such services may include:
  • in-home caregiving
  • health care navigation assistance
  • counseling and support groups
  • transportation
  • social and cultural services
  • household chore and home repair assistance
  • telephone reassurance and friendly visitors
  • legal and financial services
  • emergency response systems
