Care Management
JFCS/East Bay’s innovative care management program helps older adults live meaningful and independent lives. Our expert staff helps families choose the best care situations, navigate the health care system, manage the activities of everyday life (such as doctor appointments), encourages socialization, and secures and supervises appropriate in-home care providers. Our care managers start with a home visit to complete an in-depth evaluation of health, home safety, cognitive functioning, and social support. We then develop and implement a customized plan of care, matching the needs of each senior with services that support independence.
